In order to apply for the China Study Tour, each applicant (including guests and alumni) must complete and submit each of the following four items by the deadline of March 1, 2006. Submit all documents to

1.

 The CST2006 Application Form
  The CST application is an online form that you must complete and print. Please note that submitting the application online does not mean that your application has been received. Upon submitting the application form, you will be taken to a formatted version of the application form (containing your input data) and the terms and conditions of the tour. Please print and read the documents carefully. Once you are satisfied that the contents are accurate, please sign where required and turn in the completed form along with the other required documents.

To complete the application form, you will need access to:

  • Your passport
  • Your health insurance information
  • Your Student ID information

2.

 The Seattle University Release, Waiver, and Indemnification Agreement
  The release form is a legal agreement required by the University from all participants in an international study tour. You must read and sign the document and submit it along with your application form.

3.

 A check in the amount of $1250 made payable to Seattle University
  The fees for the Study Tour are $2800, payable in three installments. The first, somewhat sizeable, installment will be used towards the airline tickets and down payment on hotels. Please note that this check will be returned to you if we are unable to accommodate you on the Study Tour. You may pay by credit card if you so wish - please contact Ms. Joanne Dickson (206-296-5722) in Pigott 415A to make the necessary arrangements.
   

4.

Photocopy of the first two pages of your passport (must include photo page)
  Please attach a legible copy of the photo and name pages of your current passport. The passport should have at least 6 months of validity left as of the date of the tour (September 2006). If you do not have, or are renewing your passport, please include a note informing us of the date you are likely to receive it.
